Hospice Home at Transitions LifeCare

Most hospice care takes place at home, surrounded by familiar comforts. When additional support is needed, the Transitions LifeCare Hospice Home provides short-term inpatient care, helping patients remain comfortable while giving families peace of mind.

Hospice Home Care at a Glance

What is a Hospice Home?

The William M. Dunlap Center for Caring is Transitions LifeCare’s 30-bed Hospice Home, created to provide comfort, peace, and expert care when symptoms can no longer be managed at home. Unlike a nursing home or hospital, the Hospice Home is a warm, welcoming place where patients and families can focus on spending meaningful time together while our team manages the challenges of advanced illness.

Here, care is provided by an interdisciplinary team that works together to support the whole person and those who love them. Team members may include:

  • Hospice physicians and nurse practitioners
  • Registered nurses available 24 hours a day
  • Certified nursing assistants
  • Social workers
  • Spiritual care counselors
  • Bereavement specialists
  • Trained volunteers

Together, this team provides personalized care that addresses each patient’s physical, emotional, social, and spiritual needs, helping bring comfort, dignity, and support during every stage of the hospice journey.

Who Qualifies for Inpatient Care?

A physician may recommend inpatient hospice care when a patient requires a level of support that cannot be safely or effectively provided in another setting.

Common reasons for admission include:

  • Escalating pain that requires intensive management
  • Significant breathing difficulties
  • Persistent nausea or vomiting
  • Agitation, anxiety, or restlessness related to advanced illness
  • Seizure activity requiring close observation
  • Complex symptom changes requiring frequent clinical intervention

Each admission is carefully evaluated to ensure the Hospice Home is the most appropriate level of care for the patient’s needs.

Services are provided regardless of diagnosis, gender, nationality, race, creed, sexual orientation, disability, or ability to pay.

When Care Needs Change

The Hospice Home is designed to provide compassionate, around-the-clock care for patients experiencing complex symptoms or increased care needs. Because the Hospice Home is an inpatient level of care rather than a permanent residence, some patients may improve to the point that they can safely return home or move to another care setting. In these situations, our team works closely with patients and families to coordinate a smooth transition and ongoing hospice support.
