HospiceCare Eligibility


Who is eligible for Transitions HospiceCare?

Patients are eligible for Transitions HospiceCare if:

  • A physician has determined the patient has a life expectancy of six months or less, assuming their illness or disease follows the expected course
  • The patient agrees that his or her goal is comfort care rather than curative treatment
  • The patient lives in a private residence, assisted living facility or skilled nursing home in Durham, Franklin, Harnett, Johnston and Wake counties as well as Chapel Hill and Carrboro in Orange County

Admission criteria:

  • Transitions HospiceCare offers its services to patients with a limited life expectancy and their families, regardless of diagnosis, gender, nationality, race, creed, sexual orientation, disability, or ability to pay
  • Patients must have a designated attending physician who is willing to work with our hospice team. If you don’t have a physician, Transitions HospiceCare can provide one
  • Patients need a responsible caregiver at home or agree to develop an alternate place of care that meets the patient’s safety needs and is in compliance with hospice standards of care
  • Do Not Resuscitate status is not required for admission to Transitions HospiceCare
