Transitions Kids offers various levels of care designed solely around the unique needs of children (pre-natal for children with a potentially life-threatening diagnosis all the way up to age 18) facing life-limiting illness. The goal is to provide home-based supportive care services for the child and family that is focused on enhancing quality of life by reducing physical and emotional pain and distress.
