At Transitions HospiceCare we strive to help patients and families remain in their homes. Unfortunately, for a variety of reasons it is not always possible for patients to continue their hospice care at home. The William M. Dunlap Center for Caring (Hospice Home) offers three levels of care: general inpatient, respite, and short-term routine.
Hospice Home Features
Patients’ care teams work with the patient and family to determine if a stay in the Hospice Home is appropriate. Services are provided regardless of diagnosis, gender, nationality, race, creed, sexual orientation, disability, or ability to pay.
The GIP level of hospice care is of very short duration, usually less than one week. Your care team will help the patient and family determine if this is the appropriate level of care. Medicare, as well as some insurance plans, covers costs for this level of care.

– May be arranged for up to five consecutive days.
– Is provided in contracted facilities or in the William M. Dunlap Center for Caring (“Hospice Home”). Note: Hospice Home admission is based on bed availability.
– Is appropriate when the patient is medically stable, but the patient cannot be left alone, and the caregiver in the home needs to be relieved for a brief period.
– Room and board charges are usually included in insurance coverage.

If General Inpatient level of care is not needed, but the patient is at the end of life and does not have anywhere to live, or is without a needed caregiver, admission to the Hospice Home may be appropriate. Patients will be charged for room and board, as well as for medication and medical supplies that are not related to comfort or to their terminal illness.

- Unrestricted visiting hours
- Patient rooms accommodate a limited number of overnight guests
- Fully equipped family kitchen, living room and playroom
- Onsite sanctuary for spiritual needs
- Family office with wireless internet
- Laundry facilities for family members
- Landscaped outdoor areas
- Pets with proof of vaccinations can visit
- Friends and family may visit 24/7
- Nurse to patient ratio average is 5:1 and certified nursing assistant ratio is 6:1.
