Transitions LifeCare 101 – What is hospice and how can we help?

In this conversation participants will gain a deeper understanding of the story of Transitions LifeCare and the services we offer to the families and loved ones in our community who are suffering from life limiting illnesses. This presentation will help to dispel many misconceptions surrounding end of life care by describing how are services work, who is eligible, where we serve and the various care team members involved through the staff and volunteers at Transitions LifeCare.
