Designated & Special Gifts
Indigent Care Program
This program allows Transitions LifeCare to extend all of our services to patients not eligible for Medicare/Medicaid, not covered by private insurance, and without other means of paying for services. This program also helps patients and families with emergency needs, such as assistance with paying rent or a utility bill.
Make a gift today and designate it to the Indigent Care Program.
Grief Center
This program supports Transitions GriefCare, which offers individual counseling and support groups for those who have experienced the death of a loved one and helps us serve grieving families, adults, and children. Grief counseling is offered at no cost to families served by Transitions LifeCare as well as any member of the community.
Make a gift today and designate it to the Grief Center Fund.
Endowment Fund
The Endowment Fund provides an important measure of financial stability for Transitions LifeCare by creating a predictable and steady source of income for generations to come. This income stream can be used to underwrite the cost of current programs, as well as respond to future needs arising from growth in demand for hospice care and expansion of our service area.
Make a gift today and designate it to the Endowment Fund.
Transitions Kids
Transitions Kids offers various levels of care designed solely around the unique needs of children facing life-limiting illness. The goal is to provide home-based supportive care services for the child and family that are focused on enhancing quality of life by reducing physical and emotional pain and distress.
Make a gift today and designate it to the Transitions Kids Fund.
Buy a Brick
Our Raleigh campus includes two special walkways for those who want to honor their loved ones with an engraved brick. The Veterans Garden honors veterans from all military branches. The Colonnade and Memory Garden near our sanctuary includes a winding walkway featuring a fountain.
Print the Veterans Garden Brick Order Form or Memory Garden Brick Order Form to mail or bring in.
Due to supply chain issues caused by the COVID-19 pandemic, brick installations will be delayed from their usual May and November installation dates. We hope this will be resolved as soon as possible. Please email bricks@transitionslifecare.org with any questions or concerns.
Other Types of Donations
In addition to monetary gifts and brick purchases, we accept many other types of donations. Some examples include:
- Gifts of stock or appreciated securities.
- Planned or deferred gifts, like a bequest or including us in your will.
- Benefit events. You can either attend an event that benefits us or host an event and designate Transitions LifeCare to receive proceeds. Hosting an event requires completion of this form and approval by the Director of Philanthropy.
- Gifts of food or individually-sized and packaged personal care items.
For more information on types of donations, please contact us.
While we appreciate your thoughtfulness in wanting to donate items such as medical equipment (power scooters, walkers, hospital beds, etc.), clothes, pets, patient care supplies (left over tube feedings, incontinence supplies, etc.), or bedding, we are not able to accept such. Please contact a resource center in your area about these types of donations.
