Your $125 team registration includes: light breakfast, beverages, Atlantic Tire Championship tickets, and a lunch catered by Tribeca Tavern. There will be a silent auction and raffle baskets, so bring your checkbook! Team registrations only; no individual registrations accepted. No rain date.

Tom Alexander Golf Tournament benefiting Transitions LifeCare
September 18, 2017 (Monday) 9:30am
North Ridge County Club | 6612 Falls of Neuse Road. Raleigh, NC 27615
Don’t miss this opportunity to support Transitions LifeCare by participating in the 13th Annual Transitions LifeCare Golf Classic. The tournament has raised more than $377,000 for Transitions LifeCare - equating to more than 2,400 days of patient care. With your help and the support of great sponsors and wonderful volunteers, we hope to add significantly to that amount in 2017. There will be a four-person Captain’s Choice tournament, awards for top teams, a putting contest, and prizes for hole-in-ones, closest to the pin, and longest drive. Mulligans and raffle tickets may be purchased on the day of the event.
Visit the Transitions Golf Classic website for more information and registration!

Loss is “the state or feeling of grief when deprived of someone or something of value.” Grief has been described as deep sorrow, especially that caused by someone’s death, and is a natural human response to a loss. This class explores how losses and grief in our lives as professional caregivers can affect how we help those we serve as they navigate their own losses and grief throughout their life’s journey.
Schedule:
5:00-5:30pm  Hospice Home tours (optional)
5:30-6:00pm  Professional Networking and Dinner
6:00-7:00pm  Presentation

CAREGIVERS SUMMIT

Caring for a loved one can be challenging and, at times, overwhelming. Caregivers understand they have a job to do as a full-time member of their loved one’s care team. Caregiving can be shared among you, your family, your case manager, therapist or even your doctor. The Caregivers Summit provides respite, resolution, and resources as you navigate life’s transitions.

We have two conferences left this year; choose the one – or ones – that best suit your location and/or calendar.
