With services that include caregiver support and resources, hospice care, palliative care, and grief care, Transitions LifeCare can help provide a consistent continuum and even help in the assessment of the most appropriate care as a patient or loved one’s illness progresses. These services are available in Chatham, Durham, Franklin, Granville, Harnett, Johnston, Orange, and Wake counties.

Transitions LifeCare is approved as a provider for nursing continuing professional development by the North Carolina Nurses Association, an accredited approver by the American Nurses Credentialing Center’s Commission on Accreditation.

Transitions HospiceCare
