Transitions LifeCare – History
 

Our History

Founded as Hospice of Wake County in 1979, Transitions LifeCare – with its home office in Raleigh, N.C. – is the largest and most experienced, non-profit hospice provider in our region. Over the years, we’ve expanded the scope of services we provide, which now includes Transitions HospiceCare, Transitions HomeHealth, Transitions PalliativeCare, Transitions GuidingLights, Transitions Kids, and Transitions GriefCare. In addition, we’ve increased the geographic region we serve beyond Wake County to include Durham, Johnston, Franklin, eastern Chatham, and Harnett counties, as well as Chapel Hill and Carrboro in Orange County (click here to view our service area).

In 2011, our organization combined with Hospice of Harnett County, which started as an outreach committee of St. Stephen’s Episcopal Church of Erwin.

Now, as an integrated organization called Transitions LifeCare, we’re the one resource you can turn to with confidence to make sure you or a loved one receives the right care at the right time in the right setting.
